Please read the following items to ensure your registration is complete and forms are filled out correctly. The following forms and documents are for the October 16-18, 2009 Interact Symposium.
Submit ONE Intent to Participate form for EACH Interact, Mideract/Youthact, Kideract or Rotaract club attending the Interact Symposium.
Submit one Registration form for each Interact Club.
Take advantage of the VERY EARLY BIRD and the EARLY BIRD SPECIALS and send your money before the price increases (see Timeline)
Submit ONE Parental Consent Form for EVERY attendee including adults.
Ensure that the parent or guardian sign and initial the lower portion of the Registration form for any MINOR attending. This signed form must be on file with the Registrar at the time of the Symposium.
Use the Registration Change form promptly to notify the Registrar of any change in the name of attendees so that check-in can be expedited.

Rotarians and school advisors driving students to the Symposium should have a signed copy of the Registration and Parental Consent Form in their possession for every student they are providing transportation AND follow the Rotary Youth Advisory policy.
URGENT - check with your school about the School and School Board approval process for student participation in an off-campus event by May 1st (This can be a 6 week or more process).
