All of the information needed for the District Directory and RI is reported in the forms below.  Just fill in the form on page one and a form for RI is filled in simultaneously on the second page.  After completing the form, click the button marked “E-Mail Form to District Office” at the bottom of the form.  If you e-mail the form to the District Office, they will mail the form on the second page to RI for you. 

Be sure to print the completed forms for your records.  It is not possible to save the completed form on your computer.  Only a blank form can be saved. 

If you would rather fax or mail the forms, just fill them in on-line, print a two copies, and send for fax the first page to the District Office and the second page to RI.  The fax numbers and addresses are on the forms.

You can even print blank forms and fill them in by hand and mail or fax them.
